MINIMUM REQUIREMENTS:     High School Diploma or GED. Must be able to obtain Certified Behavioral Health Technician (CBHT) within 180 days as a condition of continued employment. Applicant must be able to pass a Department of Children and Families (DCF) Level II and Medicaid background screening, drug screen, and Tuberculosis (TB) test. Must have a valid driver’s license with no more than three points.

DUTIES/RESPONSIBILITIES:      Provide on-site direct behavioral health care services to mental health clients. Responsible for providing daily structure and groups to clients. Provides psychosocial rehabilitation services, prevention education, comprehensive health promotion, Mental Health (MH) and Substance Use and Addictions Disorder (SUAD) assessments (FARS) (PERFS), training and education on daily living skills, personal hygiene care, disease prevention, outreach referral services, case management, medication monitoring, and substance abuse. Provides supportive counseling, crisis management, and assists with recreational and social activities. Services are provided both individually in the home and on a group basis as determined by the treatment team. Wellness Coaches communicate all significant changes in clients’ behavior level and notable incidents to their direct report and documents pertinent information in the patient Electronic Health Record (EHR). Wellness Coaches are also responsible for ensuring safe, therapeutic environment and adhering to all safety standards, Federal and State regulations, and corporate policies. Performs other duties from time-to-time as deemed appropriate by his/her direct supervisor. Daily transport of mental health clients. Responsible for facilities and vehicle fleet maintenance. 

WORK LOCATION:      West Campus – FWB   

WORK HOURS:   Monday – Friday   

BASE WAGE:    The base wage for this position is $15.00 per hour
