POSITION SUMMARY: Bridgeway Center, Inc. (BCI) is seeking applicants that want to work with substance use clients to effectively address clients' care by screening, assessing needs, developing, monitoring, and facilitating interdisciplinary approaches.
Behavioral Wellness Clinic (BWC) offers services that help individuals link to needed psychiatric services including services for MAT. This includes direct assistance in gaining access to the MAT program, Coordination of Care (CoC), oversight of the entire case, and linkage to appropriate services. MAT Coordinator includes (1) Point of Contact (POC) for clients interested in the MAT program (2) assessment of the eligible client to determine service’s needs; (3) refer the client for needed services and set up the client with needed appointments; (4) monitoring and follow-up; and (5) enter and discharge clients and services into Florida Alcohol & Drug Abuse Association (FADAA) data base as needed. BWC educates persons served and their support system regarding diagnosis, treatment, and recovery. This position will work with clients in the Judicial system.

MINIMUM REQUIREMENTS: Bachelor’s degree in a related field. Spanish-speaking or Bilingual applicants are a plus, but not required.

Applicant must be able to pass an Agency for Healthcare Administration (AHCA) Medicaid and Florida Department of Children and Families (DCF) Level II background check, drug screen, and a Tuberculosis (TB) test. Applicant must have a valid driver’s license with no major violations and be able to be insured under BCI’s automobile policy.
JOB REQUIREMENTS: Some of the job tasks and responsibilities include, but are not limited to:
- Managing an assigned caseload of clients having substance use disorders for alcohol and/or opioid use diagnosis and for assessing client needs.
- Complete all GPRA’s/outcomes in the appropriate database.
- Provide MAT education.
- Linking clients to care and services.
- Attend staffing meetings.
- Complete invoices for the Florida Alcohol & Drug Abuse Association (FADAA).
- Order medications.
- Complete audits.
- Promote the MAT program by way of community outreach.
- Schedule MAT clients with Prescribers, Therapists, Bridgeway Health Clinics (BHC), Inc. Medical staff, and connecting them for traditional Case Management to meet/achieve any other client service needs.
WORK LOCATION: Travel between Fort Walton Beach (FWB) and Crestview (CV) locations
WORK HOURS: Mon – Fri; 8:00 a.m. – 5:00 p.m.
