Care Coordination Services
Care Coordination Services are available for Adults (ACC), Youth (YCC) and Early Childhood (ECC). These programs work with eligible people to connect them to receive the care they need to improve their physical, behavioral, and social health. The coordinators facilitate connections between healthcare and other service providers to support indiviuals and bridge gaps in the care system.
