POSITION SUMMARY: Bridgeway Center, Inc. This position is responsible for managing an assigned caseload of clients having mental illness diagnosis and for assessing client needs, developing, implementing and reviewing service plans, and working with other community resources in meeting/achieving client service needs. The Case Management department consists of services that help individuals link to needed medical, social, education, psychiatric, psychological, and other services. This includes direct assistance in gaining access to services, Coordination of Care (CoC), oversight of the entire case, and linkage to appropriate services. Case Management includes (1) assessment of the eligible client to determine service needs; (2) development of an individualized integrated service plan; (3) referral and related activities to help the client obtain needed services; (4) monitoring and follow-up; and (5) evaluation. The Case Management department educates persons served and their support system regarding diagnosis, treatment, and recovery.

MINIMUM REQUIREMENTS: Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and have a minimum of one year of full time or equivalent experience working with children with serious emotional disturbances.

Applicant must be able to obtain or currently have Florida Certification Board Credentialing as a Targeted Case Manager for both Adults and Children. If applicant does not have certification, the applicant must obtain certification within six (6) months of hire. Please include a resume with the application. Applicant must be able to pass a Level II DCF criminal background check and drug screen. Applicant must have a valid driver’s license with no major violations.

BASE WAGE: sThe base wage for this position with 0-5 years of experience is $15.00 per hour and for 6-10 years of experience is $16.00 per hour. The maximum years of experience that BCI gives credit for is ten (10) years.
